Employee Injury

When a job related injury occurs, the employee's supervisor must send a completed Supervisor's Report of Injury form (Word document opens in new window) to the campus nurse in Health Services.

Student Incident/Injury

When a Student incident/injury occurs on campus or at a college sponsored event, a Report of Student Incident form (PDF document opens in new window) must be completed by the TCC employee in attendance and sent to the campus nurse in Health Services.
