How to Apply
Apply online at the U.S. Department of Veteran Affairs for educational benefits.
OR
Submit one of following forms in person to a veteran and military benefits representative at a campus Registrar (all are PDFs and open in new window):
- Application for VA Educational Benefits form 22-1990
- Request for Change of Program or Place of Training form 22-1995
- Application for Survivors' and Dependents' Educational Assistance form 22-5490
- Request for Change of Program or Place of Training Survivors' and Dependents' Educational Assistance form 22-5495
VA benefits applicants must also submit the following forms to the veteran and military benefits representative in the Registrar's Office at the campus they plan to attend (PDFs open in new window):
- Certificate of Responsibility and Compliance.
- VA Information Sheet (required every semester for certification).
Veteran certifications are completed by the Registrar's office.
